Coverage details
  • State plan, private plan option.
  • Private plans may be insured or self-insured and must be equal to or in excess of public plan benefits.

Symetra does not offer an insured plan or ASO services for an employer’s self-insured plan.

Covered employers

All employers who have or have had one or more employees during any day of the current or preceding calendar year.

Covered individuals

Employees must:

  • Be unable to work due to injury or illness that is not related to work or a car accident;
  • Be in treatment with a duly authorized physician or chiropractor.
  • Have earned at least $150 in a covered employment during her or her base year.

Certain domestic servants, students employed by a school or college, government or non-profit organization employees are excluded.

Contribution amount
  • State plan rate is 0.6%.
  • Employee contribution:
    • 0.3% of the first $9,000 of employee’s annual earnings.
  • Employer Contribution:
    • 0.3% of the first $9,000 of employee’s annual earnings.
    • Employer has option to pay all or a portion of the total amount (0.6%).
Waiting period
  • Eighth consecutive day of disability; or
  • First day of hospitalization, if earlier.
Benefit calculation

65% of weekly earnings.

Additional benefits
  • Death benefit in the amount of $4,000.
  • Dismemberment benefits ranging from $2,000 to $4,000.
Maximum weekly benefit amount

$113 per week.

$55 per week for agricultural workers.

Minimum weekly benefit amount

$12 per week.

Other income amount offsets

100% backdoor offset with any wages paid during disability.

Maximum duration

Not more than 26 weeks during any disability period or any consecutive period of 52 weeks.
